Transitional Care - Homeward Bound

Homeward Bound Care is a new initiative on the Sunshine Coast, with our services coordinated by Sundale to achieve the best possible transition for you from hospital to home. The aim of the program is to provide additional support to those who have been in hospital, but are not quite ready to return home. We provide tailored programs of rehabilitation, specifically designed accommodation, and in-home care services to support your recovery. Indeed we can even help you find more appropriate accommodation options for your needs.

The Homeward Bound Care service is provided at two levels, with residential care provided at our Nambour site, and in-home care provided at your Nunyara own home. The overall program is available for a maximum of 12 weeks and can be either or a combination of the residential and community based services.

Nunyara Care 

Based at Sundale in Nambour, Your Nunyara House is a wonderful new facility we offer the community, which was specifically designed and built for this purpose. This facility has been designed for people who have been discharged from hospital but still require
support and care before returning home. Our Nunyara House is a relaxing and supportive environment where you have your own queen sized bedroom and en-suite and you have the choice as to the type of menu and food preferences. The care model is one of "supported self care" where team members are only on site as and when required, with the balance of your time being spent with family, friends, or your house mates.

The Nunyara House is a secured facility with all possible safety measures in place. It truely is a "home away from home" but is also a pathway in your homeward bound journey to recovery.

In - Home Care

Our services are tailored to your needs, and we will work with you and yours on the best options for your rehabilitative experience. Allied Health and care teams will come to your home to hep you with your tailored exercise program, or you can access rehabilitation support in our Sundale Therapy Centre, which includes a marvellous hydrotherapy pool to sooth away aches and pains. You can make your own way to the Centre or we may be able to assist you with transport through our therapy centre bus.
