Admission process 

5 steps for aged care

To determine the right type of care for you and the best level of support, you’ll need to go through a government-funded assessment via My Aged Care. Here’s an overview of what the journey will look like.

First enquiry  

Check your eligibility for an assessment of aged care services with My Aged Care. You can do this online or by calling 1800 200 422. If you’d prefer to speak with us first, get in touch.

Assessment 

If your application is successful, an ACAT (Aged Care Assessment Team) professional will visit your home to understand your needs. After your assessment, you’ll find out if you’re eligible for government funded services – and if so, which ones. To make this assessment as smooth as possible, you should prepare for your assessment.

Choose a provider

Once your needs have been assessed, and if you’re eligible for services; you can start engaging a care provider (like us!) to find the best support for your needs.

Finalise your care plan

We’ll work with you to create a personalised support plan, whether that’s through in-home care, or at one of our residential aged care communities.

Start receiving care 

Once your support plan has been finalised, and fees or funding have been organised, you can start receiving care at home or prepare to move into a Sundale residence!
